By appearing for an in-office consultation, you acknowledge the highly contagious nature of Covid-19 and voluntarily assume the risk of exposure or infection by attending and that such exposure may result in personal injury, illness, disability, or death. You agree and understand the risks associated with Covid-19 and accept them in order to attend an in-office consultation. You agree to comply with all Covid-19 related procedures that may be implemented at our office including, but not limited to, mask wearing and social distancing requirements, to protect as much as possible the health and safety of all participants in the consultation.

You agree to be the only party present, this includes your spouse if he/she is also our client/pending client. At this time we are not allowing any friends, or family members to accompany you to your in-office consultation. You further herby release and waive all claims and rights to bring suit against Katina Pantazis, PA, its employees, officers, directors, and/or agents in connection with the exposure, infection and/or spread of Covid-19 related to attending an in-office consultation. You agree that, if you have the Covid-19 infection or are experiencing any of the known symptoms of Covid-19, you will not attend your in-office consultation, and upon request, may obtain a refund of the in-office.
